Diabetes Self Management ProgramFamily Resource Center at the Ocean ClubThe Focus of Diabetes education at SOCH is on both the person with diabetes and their family.
It centers around blood glucose monitoring, nutrition therapy, exercise and medication management of diabetes. Participants in this program are give one-on-one time with a Certified Diabetes Nurse and Certified Diabetes Dietician educators. In addition to personal consultations , participants are offered an educational series of classes designed to expand their knowledge of diabetes and improve their quality of life. Lifeline ProgramFamily Resource Center at the Ocean ClubThe Lifeline Service can help your loved one continue living independently and with increased confidance at home
Lifeline is a simple wireless device worn as a pendant or wristband that allows the wearer to summon help at the push of a button, 24 hours a day, 356 days a year. In case of emergency such as a fall, the subscriber pushes the button activating the CarePartner Communicator, which connects to a trained response associate who quickly assess the situation and dispatches assistance.
